Privacy Statement GDPR (General Data Protection Regulation)

At Integrative Health Clinic, I place great importance on your privacy. I take the protection of your personal data seriously and ensure that your information is treated confidentially and with the greatest possible care. The processing of personal data takes place in accordance with the General Data Protection Regulation 2016/679 (GDPR).

By using my website and/or my services, you indicate that you agree to the terms set out in this privacy statement. This privacy statement applies to all services provided by Integrative Health Clinic. Before scheduling your first appointment, I ask that you provide written consent to this privacy statement by means of a signature.

Record Keeping

For optimal treatment, it is necessary that I, as your treating therapist, maintain a client file. This is also a legal requirement under the Medical Treatment Agreement Act (WGBO/Wkkgz).

Your file contains notes regarding your state of health, data about the examinations performed, and proposed treatments. In addition, the file includes data necessary for your treatment that, after your explicit consent, may have been obtained from other healthcare providers, such as your general practitioner, osteopath, dietitian, etc.

Protection of Privacy

At Integrative Health Clinic, I do everything possible to safeguard your privacy. This means, among other things, that I:

• Handle your personal and medical data with care

• Ensure that unauthorized persons do not have access to your data,

• Am bound by a legal duty of confidentiality (medical professional secrecy)

• Act in accordance with the guidelines of the WGBO and the Personal Data Registration Act.

The data from your file may also be used for the following purposes:

• To inform other healthcare providers, for example when the therapy has been completed or in the case of referral to another practitioner. This will only occur with your explicit consent

• To share your personal details with pharmacies or other supplement suppliers when you ask me to order the recommended supplements on your behalf

• For use by a substitute therapist during my absence

• For anonymized use during intercollegial review

• For financial administration purposes, so that I can prepare an invoice.

If I wish to use your data for any other reason, I will first inform you and request your explicit consent.

Right of Access, Correction and Deletion of Personal Data

As a client, you have the right to access your stored personal data and to correct any inaccuracies. You also have the right to have your personal data deleted, in accordance with the conditions set out in the GDPR and the WGBO. Please contact me if you wish to exercise these rights.

Retention Period

The data in your client file will be retained for 20 years, as legally required under the WGBO.

Privacy on the Healthcare Invoice

The healthcare invoice you receive contains the information required by health insurers, so that you can submit the invoice for reimbursement. This information includes:

• Your name, address, and place of residence

• Your client number and/or date of birth

• Insurance and/or policy number

• The date of the treatment and claim number

• A short description of the treatment

• The performance code

• The cost of the consultation.

Cooperation with Other Organizations

From my practice, I collaborate with the following organization in relation to the processing of your data, with which a data processing agreement has been established: Software company MijnDiAd
